EMERGENCY CARDS

These cards are sent home the first day of each school year for parents/guardians to be completed signed and returned on the second day of school. This vital information is the only link between the nurse and the parent/guardian in the event of an emergency or illness. Please keep the nurse updated on any changes of information on this card. Do not list non-custodial parents unless it is okay for them to be contacted and/or pick up your child.


VISION SCREENING

Vision screening is performed for Kindergarten, Third graders, and any newly enrolled students. Parents or teachers may refer any student displaying visual difficulties for a vision exam.

HEARING SCREENING

All students in grades 1 and 4 have their hearing checked with an Audiometer. This screening is available to students in other grades who display symptoms of hearing difficulties, based on teacher referral.
