Administering Medication

Requirements for administering medication to students by school personnel are as follows:

1.  The prescribed medication must be in the original container bearing the original pharmacy label which includes the directions from the physician, name of the medication, date, and student's name.  The non-prescription (over-the-counter) medication must be in the original container.  Written instructions from the parent/guardian must accompany any medication to be given at school.

2.  The parent or guardian must sign an Authorization for Administering Medication by School Personnel card.  This form is provided by the nurse and kept on file in the Nurse's Office.
