FREE SCHOOL PHYSICALS & IMMUNIZATIONS FOR UNINSURED K-12 STUDENTS

School Physicals and Immunizations will be provided at No Charge for school-age children. (No preschool/HeadStart physicals will be offered.) These services are available for any school-aged child without insurance. The student must be accompanied by a parent or legal guardian and they MUST have their current immunization record. Various agencies will be on-site providing health education materials. Assistance with Hoosier Healthwise, HIP and Medicaid enrollment will be available by appointment.

"On the first day of school, students were given a copy of the
Student Conduct: Rights, Responsibilities, Discipline Policy And Student Rules. Students were required to sign for the receipt of these rules. If you did not have a chance to review the rules and would like to have a copy, please call the school office." per Mrs. Weber
